At Care is There Geriatric Care Management we are excited to see JABA helping senior citizens transition safely and successfully from hospitals and skilled rehabilitation centers back to their  their own homes and independent living communities.
Different from other services like home health or physical therapy, the program coaches its participants to manage their medical conditions and connects them to helpful community resources with the goal of preventing unnecessary return trips to the hospital.Â
If you, your loved one, client or resident is currently in the hospital or a skilled rehabilitation center or is scheduled for admission soon, you can contact JABA now to plan a successful transition home! Simply contact Care Transitions Coordinator, Mary Ellen Hasenfus at mhasenfus@jabacares.org, or by calling 434-817-3556. You may be eligible to receive the services at no charge, and unlike Medicare-based services, you don’t need to be homebound to qualify for the program.
Support provided over a one month period from a JABA Care Transitions Specialist includes:
- A visit while you are in the hospital to help you plan for discharge.
- Visits to your home within 1 to 3 days of discharge to review your medications, identify red flags that you need medical assistance, schedule follow-up medical appointments, and prepare for your doctor visit. Support services may extend over a longer period as determined by your Transitions Specialist.
- Weekly calls over the next few weeks to continue supporting you while you recover.
