POLST forms are part of a nationwide program that ensures your loved ones or clients will receive the care they want. The names vary by state. In Virginia, it is called a POST Form. If your loved one or client is seriously ill or frail, a POST Form can help prevent unwanted emergency medical care.
POST stands for Physician Orders for Scope of Treatment. A Virginia POST form contains standing medical orders applicable across various care settings and actionable by emergency medical personnel.
